Understanding Fasting Insulin

Your fasting glucose is normal. Your HbA1c is normal. But you're exhausted after meals. You struggle with cravings. Your weight has slowly increased despite making healthy choices. You feel like your metabolism has changed.


One of the most common things I hear in practice is: "My labs look fine, but I know something isn't right."


This is where fasting insulin can sometimes provide additional information.


What does fasting insulin tell you?


Fasting insulin measures the amount of insulin in your blood after an overnight fast. While fasting glucose shows how much glucose is circulating, fasting insulin can provide additional information about how much insulin your body may be producing to maintain that glucose level. In some people, insulin levels may change while fasting glucose and HbA1c remain within the reference range.


Glucose tells us how much sugar is in the bloodstream at a specific moment. Fasting insulin provides additional information about the insulin response involved in maintaining that glucose level.


Two people can have identical fasting glucose levels while having very different insulin patterns. This is one reason fasting insulin may sometimes provide additional metabolic context while glucose markers are still within the reference range.


Why This Marker Matters


One of the reasons I appreciate fasting insulin so much is that it may provide insight into metabolic changes earlier in the process.

Our bodies are remarkably adaptive. For many years, the body may compensate by producing more insulin in order to maintain normal blood sugar levels.


As a result, someone may experience fatigue after meals, increasing sugar cravings, changes in body composition, rising triglycerides, worsening cholesterol patterns, and slowly increasing HbA1c over time, long before fasting glucose becomes abnormal.


This is one reason fasting insulin can be such a valuable piece of the puzzle.


What a "Normal" Glucose Result Can — and Can't — Tell You

A normal fasting glucose or HbA1c does not necessarily tell us everything about insulin regulation. In some people, glucose can remain within the reference range while insulin production has increased to help maintain it.

This is one reason fasting insulin can sometimes add useful context. Glucose tells us about the amount of glucose circulating in the blood. Fasting insulin provides different information about the insulin response occurring at that time.

Two people with similar fasting glucose values may have different fasting insulin levels. That does not make fasting insulin a standalone diagnostic test, but when it is interpreted alongside HbA1c, lipids, symptoms, family history, medications, and other clinical information, it may help provide a fuller picture of metabolic health.


Why It's Not Part of a Standard Panel


Routine screening labs are incredibly valuable and designed to identify disease efficiently across large populations. That is exactly what they are intended to do. Fasting insulin simply serves a different purpose. It may provide additional information about metabolic patterns before overt disease develops.

This does not mean routine laboratory testing is inadequate or that something was missed. Rather, it highlights that different tests answer different questions.


One of the biggest misconceptions I encounter is the idea that "normal labs" automatically mean everything is functioning optimally. A result within the reference range does not always provide the full picture of metabolic health, particularly when symptoms or other risk factors are present.


Why Symptoms Still Matter


Laboratory values are important. Symptoms matter too.

Fatigue. Cravings. Brain fog. Difficulty losing weight. Energy crashes after meals.

These experiences often provide valuable context when interpreting laboratory results. This is one reason I rarely look at fasting insulin in isolation.


Instead, I look at patterns. How someone feels. Family history. Lifestyle factors. Other laboratory markers. Because health rarely exists in isolated categories, and a number without that context can be genuinely misleading in either direction, reassuring when it shouldn't be, or alarming when it doesn't need to be.


What Fasting Insulin Can Suggest


There is no single "perfect" fasting insulin value that applies to everyone. Interpretation depends on age, symptoms, medications, family history, fasting glucose, HbA1c, lipid patterns, and overall clinical context.

Many practitioners focus less on one isolated number and more on trends over time and how the result fits into the broader picture. This is one reason internet searches can only provide limited guidance. Laboratory interpretation is always individualized.


A Note on PCOS


Fasting insulin also comes up frequently in the context of PCOS (recently renamed PMOS, or polyendocrine metabolic ovarian syndrome, to better reflect its full hormonal and metabolic picture), since insulin resistance is a common, though not universal, feature of the condition. If PCOS is part of your picture, fasting insulin can be a genuinely useful piece of the puzzle, alongside the hormonal and cycle-related markers your practitioner may already be tracking.


What This Can Look Like in Real Life


In practice, I frequently work with individuals who have normal fasting glucose but increasing HbA1c, experience significant fatigue after meals, feel like they are doing everything right but continue gaining weight, have a strong family history of diabetes, notice increasing sugar cravings and changing cholesterol patterns, or feel frustrated because their labs appear "normal" despite not feeling well.


Sometimes fasting insulin helps provide additional context that allows us to better understand these patterns.


A Common Pattern


One example of how this often unfolds: someone had been told, year after year, that her labs were completely normal, despite feeling foggy, fatigued, and increasingly frustrated after meals.


Her fasting glucose remained normal, but she felt like her body was changing.


By looking at her symptoms, family history, lifestyle factors, and metabolic markers together, we were able to identify patterns that helped guide her nutrition and lifestyle plan.

Over time, she experienced improvements in energy and gained a much better understanding of how her body responded to food, sleep, stress, and movement (real client outcome, 2023).


How We Use This Marker in Practice


Fasting insulin is rarely interpreted by itself. Instead, it becomes one piece of a much larger picture, alongside fasting glucose, HbA1c, triglycerides, HDL cholesterol, c-peptide, continuous glucose monitor data, family history, and symptoms. Some practitioners also calculate additional metrics, such as HOMA-IR, to better understand insulin sensitivity patterns.


Because our bodies function as interconnected systems, a single laboratory value rarely tells the entire story. Instead of asking "What does this one number mean?" I often encourage clients to ask "How does this number fit into my overall picture?" This often leads to much more meaningful insights.


When an active nutrition or lifestyle plan is underway, periodically rechecking markers can help identify trends without over-testing. Because fasting insulin can fluctuate, trends over time are often more informative than any single value. An overnight fast of approximately 8-12 hours is generally recommended prior to testing.


How to Ask for This Test


If you are interested in fasting insulin, consider discussing it with your physician during your next routine visit. You might say: "Given my family history and symptoms, would it make sense to add a fasting insulin level to my laboratory work?"


Although fasting insulin isn't routinely included in standard screening, I believe a baseline measurement can provide useful context for many adults, particularly when interpreted alongside fasting glucose, HbA1c, lipid markers, symptoms, and family history. Whether and how often to repeat testing should be individualized.


A Note on C-Peptide as an Alternative Marker


C-peptide is another marker worth knowing about, and some practitioners consider it a useful reflection of your own insulin production, and it tends to be more stable than fasting insulin.


The trade-off is largely practical. C-peptide testing tends to cost more and may be less consistently covered by insurance unless diabetes is already present.


Both fasting insulin and c-peptide have strengths and limitations. The most appropriate option depends on your individual health history and the specific question you are trying to answer.

Why Catching This Early Matters


One of the most encouraging things about metabolic health is that many changes occur gradually. This creates an opportunity.


Small Habits, Meaningful Change

Because early metabolic changes may occur while routine glucose markers remain within reference ranges, they may not always be apparent from standard screening alone.


Identifying patterns earlier often allows for meaningful interventions before more significant metabolic changes develop. Small, consistent habits can make a substantial difference over time.


Increasing protein intake. Improving sleep. Walking after meals. Building muscle. Increasing fiber gradually. Managing stress.


These changes may seem simple, but together they can meaningfully influence metabolic health.


Progress rarely requires perfection. Often, it simply requires understanding your body's unique patterns and knowing where to focus.


Common Questions About Fasting Insulin


Can fasting insulin be elevated even if glucose is normal? Yes. This is one reason fasting insulin may sometimes identify patterns earlier than fasting glucose alone.


Do I need to fast? Generally, yes. An overnight fast of approximately 8-12 hours is typically recommended.


Does an elevated fasting insulin mean I have diabetes? No, not necessarily. It simply provides additional information regarding how the body may be regulating blood sugar.


Should everyone have fasting insulin tested? I believe having a baseline fasting insulin can provide useful context for many adults, even when fasting glucose and HbA1c are normal. It isn't currently part of routine screening for everyone, and testing decisions should still be individualized with your healthcare team.


Is this test expensive? It varies, but fasting insulin is typically inexpensive compared to more specialized testing, and many labs include it easily alongside routine bloodwork once it's specifically requested. Fasting insulin, along with other markers, can also be ordered directly through functional lab testing if that route fits your situation better.


What if my fasting insulin comes back elevated? An elevated result isn't cause for alarm on its own, it's simply useful information to build a plan around, usually starting with the same foundational changes, protein-forward meals, movement after eating, and consistent sleep, then reassessing the trend over time. Consistency matters here, addressing these patterns early is generally easier than waiting until they've progressed further.


Putting It Together


Fasting insulin is a relatively simple laboratory marker that can sometimes provide valuable additional information about metabolic health. It is not a standalone diagnostic tool. Nor does it tell the entire story.

However, when viewed alongside symptoms, family history, lifestyle factors, and other laboratory markers, it can help create a more complete picture.


Sometimes understanding how hard the body may be working behind the scenes is the missing piece that finally helps explain why someone does not feel like themselves.

And because metabolic changes often occur gradually, identifying these patterns earlier can provide an opportunity to make meaningful, sustainable changes that support long-term health.
