You eat lunch, and by 2 or 3 pm you're wiped out. Not sleepy, completely drained.
Maybe you're reaching for another cup of coffee or something sweet just to make it through the afternoon. Perhaps you're feeling hungrier than you think you should, struggling with brain fog, or wondering why your energy and metabolism don't feel like they used to.
One of the most common things I hear in practice is: "I feel like I'm doing everything right, but my body just isn't responding the same anymore."
For some people, these changes happen gradually. Others feel like their body changed almost overnight.
While there are many possible reasons for these symptoms, insulin resistance is one possibility that deserves a closer look, because it often develops quietly for years before it ever appears on routine lab work.

One of the challenges with insulin resistance is that the symptoms often seem completely unrelated.
Energy crashes. Cravings. Poor sleep. Brain fog. Increasing abdominal weight. Difficulty recovering from exercise. Changes in cholesterol.

Many people don't realize that these experiences can all be connected. Our bodies are incredibly interconnected. Blood sugar influences energy. Energy influences movement. Movement influences muscle mass, and muscle mass plays an important role in metabolic health.
This is why looking at patterns over time often provides much more insight than focusing on any one symptom in isolation.
What Insulin Resistance Actually Is
Insulin's job is to help move glucose from the bloodstream into our cells where it can be used for energy.
When our cells become less responsive to insulin, the body compensates by producing more of it. This compensation is your body doing exactly what it's designed to do, protecting you from dangerously high blood sugar, but it comes at a cost over time.
This compensation can maintain "normal" blood sugar levels for many years. Meanwhile, insulin itself may be quietly increasing. This is one reason why someone can feel unwell long before routine lab work suggests there is a problem.
Why Insulin Resistance Matters Even Before Diabetes
Insulin resistance matters well before it ever progresses to type 2 diabetes, or even prediabetes. Long before blood sugar itself becomes a concern, elevated insulin has been linked to a wider set of health patterns worth understanding: increased cardiovascular risk, non-alcoholic fatty liver, persistent fatigue, and broader metabolic strain on the body.
This is part of why addressing these patterns early is so valuable. The goal isn't just to prevent a future diabetes diagnosis, though that matters too, it's to support cardiovascular health, liver health, energy, and overall metabolic resilience well before any single marker crosses into diagnosable territory.
The Early Signs of Insulin Resistance Most People Miss
Some of the early signs of insulin resistance may include:
● Energy crashes 2-3 hours after eating
● Intense cravings for sugar or refined carbohydrates
● Feeling hungry shortly after meals
● Brain fog or difficulty concentrating
● Increasing weight around the midsection despite making healthy choices
● Darkened, velvety skin around the neck, underarms, or knuckles (acanthosis nigricans)
● Waking up hungry, shaky, or wired during the night
● Feeling like your body simply doesn't respond to your efforts the way it once did

Two related patterns are worth mentioning even though they're not something you'd notice day to day: elevated triglycerides and declining HDL cholesterol. These are lab findings rather than felt symptoms, but they often show up on a standard lipid panel years before glucose or insulin numbers move, making them an early clue worth asking your doctor about.
Signs of Insulin Resistance in Women
Insulin resistance can show up somewhat differently for women, often overlapping with hormonal patterns. Irregular or unpredictable menstrual cycles, and conditions like PCOS, are closely linked to insulin resistance for many women, since elevated insulin can directly affect ovarian hormone production. Changes in hair growth patterns or acne, particularly along the jawline, can be part of this same picture.
During perimenopause, some women notice these signs becoming more pronounced, hot flashes and night sweats that seem to track with blood sugar swings, or new cravings and midsection changes that feel different from earlier in life. None of this means insulin resistance is guaranteed to be the cause, but it's a pattern worth mentioning to a practitioner alongside anything else you're noticing.
What This Can Look Like in Real Life
In practice, I frequently work with individuals who skip breakfast and rely on coffee to get through the morning, feel tired after meals despite eating "healthy," experience increasing sugar cravings in the afternoon, wake up in the middle of the night and struggle to get back to sleep, have gradually gained weight around the midsection despite exercising regularly, or notice that their cholesterol or HbA1c has slowly started creeping upward.
Individually, these symptoms may not seem particularly significant. Together, however, they often suggest that blood sugar regulation deserves a closer look.
Why Standard Screening Often Misses This Early
Fasting glucose is often one of the last markers to change. Fasting insulin, and in some cases C-peptide, may begin changing years before fasting glucose becomes abnormal, but it's not routinely included in many annual screening panels unless specifically requested.
This does not mean your doctor missed something. Routine screening labs are incredibly valuable and designed to identify disease efficiently. That's a meaningful design difference, not a flaw, screening panels are built to catch disease efficiently across large populations; they were never built to flag a slow, multi-year metabolic shift in one specific person.
Part of why this happens so quietly is that our bodies are remarkably adaptive. For years, the body can compensate by producing more insulin to keep blood sugar within a normal range, which is exactly why symptoms so often show up before any lab value does. Symptoms tend to come first. Lab changes tend to come later.
A Common Misconception Worth Addressing
A lot of people assume that if their doctor did not flag anything, insulin resistance cannot be part of their picture.
That is not necessarily true. It may simply mean that the specific marker was not tested, or that the results still fall within a broad reference range.
This is one reason why trends, symptoms, and lifestyle patterns can provide valuable additional information.
What To Do If You Recognize Yourself Here
The good news is that these patterns often improve meaningfully with lifestyle changes, particularly when addressed early. Some simple starting places include:
● Prioritize protein. For many people, this may look like eggs with fruit and Greek yogurt at breakfast, adding protein powder to a smoothie or coffee, or including fish, chicken, cottage cheese, tofu, beans, or other protein sources at meals.
● Increase fiber gradually. Vegetables, berries, legumes, nuts, seeds, and whole foods can help support satiety and blood sugar regulation.
● Move after meals. Even a 10-minute walk after eating can meaningfully improve glucose regulation.
● Prioritize sleep. Poor sleep can significantly worsen insulin sensitivity.
● Track patterns. A food and symptom journal, or for some individuals, an over-the-counter continuous glucose monitor, can provide valuable insights.

There is no one perfect diet. The goal is understanding your unique physiology and identifying strategies that work for your body.
Many people are surprised by how much better they feel from relatively small, sustainable changes. Improving protein intake, taking a short walk after meals, or getting slightly more sleep may not seem dramatic, but over time these small changes can create meaningful improvements in energy, cravings, and overall metabolic health.
How This Shows Up Differently From Person To Person
Family history, stress levels, sleep quality, muscle mass, activity levels, medications, hormonal transitions, including perimenopause, and nutrition all influence how insulin resistance develops. Hormonal shifts during perimenopause in particular can affect insulin sensitivity directly, which is often why these symptoms become more noticeable during that transition specifically.
For one person, cravings may appear first. For another, it may be increasing cholesterol, fatigue, or changes in body composition. Someone with a strong family history may see this pattern emerge earlier and respond more slowly to changes. Someone under significant chronic stress may notice cravings and sleep disruption well before any change in body composition at all.
This is why individualized care is so important.

Looking at the Bigger Picture
One of the things I love most about functional nutrition is helping people connect the dots. Insulin resistance rarely develops because of one single factor.
Sleep. Stress. Muscle mass. Nutrition. Movement. Genetics. Hormonal changes. All of these factors influence metabolic health.
Rather than asking "What is the one thing causing this?", I often encourage clients to ask "What patterns may be contributing to how I feel?" This often creates a much more helpful and empowering conversation.
A Few Questions Worth Asking
Can I have insulin resistance even if I'm not overweight? Yes. It's influenced by genetics, muscle mass, stress, sleep, and many other factors, not body size alone, and it shows up across a wide range of body types.
Is this the same thing as prediabetes? They're related but not identical. Prediabetes is typically defined by fasting glucose or HbA1c crossing a specific threshold; insulin resistance is often present years before either of those numbers moves.
Do I need special testing to know for sure? Not necessarily to start. Many of these patterns can be addressed through food, movement, and sleep changes while you and your practitioner decide together whether additional testing makes sense.
Putting It Together
Insulin resistance often develops quietly and gradually, which is exactly why these early signs matter.
Fatigue. Cravings. Brain fog. Poor sleep. Changes in body composition. These symptoms are not necessarily something you simply have to accept as part of aging or living a busy life. They may be your body's way of communicating that something deserves a closer look.
The earlier these patterns are recognized, the more opportunity there is to make sustainable changes that support long-term health.
