You eat something. Hours later, or sometimes even days later, you don't feel quite right.

Maybe it's bloating, fatigue, brain fog, headaches, joint aches, skin changes.


One of the most common things I hear is: "I don't know if this is a food sensitivity, an intolerance, or if I'm just imagining things."


You're not imagining it. And you're certainly not being dramatic. If you've ever wondered why a food that never bothered you before suddenly does, or why your reactions seem to shift over time without your diet actually changing, this is exactly why.


Food reactions can be complicated. Part of the confusion comes from the fact that food sensitivities and food intolerances are genuinely different processes in the body.


And understanding food sensitivity vs food intolerance often changes what actually helps. Two people can react to the exact same food for completely different reasons, one from an enzyme gap, another from an immune response, which is exactly why the mechanism matters more than the symptom list alone.


What Is A Food Intolerance?


Food intolerance is typically a digestive issue. The body lacks something it needs to properly break down a food.


The classic example is lactose intolerance. Without enough lactase enzyme, lactose is not properly digested, which may lead to gas, bloating, diarrhea, abdominal discomfort.


This is primarily a digestive capacity issue. Not an immune issue. Most people have never had anyone walk them through this distinction before, they've just been told to "figure out their trigger foods" and left to guess.


What Is A Food Sensitivity?


Food sensitivities involve the immune system. Approximately 70% of the immune system resides in the gut.


I often explain this to clients using an airport analogy. Everything you swallow passes through security. If your immune system recognizes something as safe, it allows it to pass through. If not, it may initiate an immune response.

Because this involves immune activity and inflammatory signaling, symptoms may appear hours or even days later. This delayed timing is one reason food sensitivities can be difficult to identify on your own. Sensitivities can develop to almost any food, though gluten, dairy, eggs, soy, corn, and certain additives are frequently discussed triggers. The specific list is different for every person, which is part of why generic elimination lists circulating online don't always solve the problem the way individualized testing and reintroduction can.

Symptoms Are Information


One of the reasons food reactions feel confusing is because symptoms often do not seem connected.


Someone may experience fatigue, headaches, eczema, joint aches, brain fog without immediately thinking about food.


But symptoms are information. Your body may be trying to communicate that something about the gut environment needs additional support.

Food Reactions Are Not Always About The Food Itself


One of the biggest misconceptions is believing that the food itself is always the problem. Sometimes the environment has changed.


Food reactions may become more noticeable after illness, antibiotics, chronic stress, sleep deprivation, digestive dysfunction, hormonal changes, microbiome disruption.


That can disrupt the gut environment and cause new food reactions, including stress, antibiotics, poor sleep, hormones, and more.

This is one reason some people suddenly react to foods they previously tolerated, sometimes foods they'd eaten comfortably for years.


The Gut Barrier Is An Ecosystem


The intestinal barrier is much more than a single wall. It is an ecosystem composed of intestinal cells, mucus, digestive enzymes, antibodies, immune cells, beneficial microbes.


When one part becomes disrupted, the entire environment may be affected. This is why digestive symptoms rarely occur in isolation. A gut that's already under strain simply has less capacity to buffer anything extra, whether that's a new food, a stressful week, or a rough night of sleep.

What Is Intestinal Permeability?


Between intestinal cells are structures called tight junctions. These junctions allow nutrients to pass through while keeping potentially harmful particles out.


Inflammation may alter these junctions and increase intestinal permeability. This is sometimes referred to as "leaky gut."

Increased permeability may allow particles to enter circulation that would normally remain within the digestive tract, which is part of why an irritated gut can seem to react to more and more foods over time.


Why This Matters


This is not simply about bloating. Digestive health influences inflammation, immune function, energy, mood, skin, sleep, autoimmune activity.


Again, the body functions as an interconnected system.

Ongoing immune activation isn't just uncomfortable in the moment. Ongoing intestinal permeability has been connected in the research to broader inflammatory patterns, and for some people, to increased susceptibility to autoimmune activity over time. This doesn't mean everyone with food reactions is heading toward an autoimmune condition, most aren't, but it's part of why this is worth addressing directly rather than working around indefinitely with an ever-narrowing list of foods.


Why Stress Matters Too


Stress affects digestion more than most people realize.


The nervous system influences stomach acid production, digestive enzymes, motility, immune activity, intestinal permeability.


This is one reason food reactions often worsen during stressful periods, even when the food itself hasn't changed at all.


A Structured Way To Approach Food Sensitivities


This is where I often think about digestive health using a 5R framework: Remove, Replace, Reinoculate, Repair, Rebalance.


This isn't a fixed protocol applied identically to everyone. It's the structure I use to organize the work, personalized based on what's actually driving your specific picture, your triggers, your history, and how your gut is currently functioning.


The goal is rarely avoiding foods forever. The goal is restoring resilience.


A Note on Medications and Your Gut Lining


Certain medications, including NSAIDs and proton-pump inhibitors, have also been linked to increased intestinal permeability. This isn't a reason to stop anything on your own, it's worth a conversation with your physician if this applies to you, especially if gut symptoms have been part of your picture.


What This Can Look Like in Real Life


In practice, I frequently work with individuals who react to foods that never used to bother them, who've had inconclusive or conflicting results from commercial sensitivity panels, who've been managing joint aches or brain fog for years without ever connecting them to food, or who feel like their list of "safe" foods keeps quietly shrinking.


A Common Pattern


This is a pattern that comes up often: someone came in with fatigue, bloating, and joint aches, with nothing seeming immediately connected to food.


Through a structured elimination and reintroduction process, a daily trigger was identified.

As inflammation improved and digestive support was introduced, symptoms improved substantially.

Common Questions


Can food sensitivities change? 

Sometimes, yes, once the underlying gut environment is supported and the trigger has been removed for a period, many people are eventually able to reintroduce it without a reaction.


Are IgG panels accurate?

They may provide information but should be interpreted cautiously, the research on their reliability is mixed, and results are best read alongside your symptoms and history with a practitioner, not as a standalone answer.


Will I need to avoid foods forever? 

It depends. The goal is often improving the environment so foods may eventually be reintroduced.


How is this different from a typical elimination diet? 

A generic elimination diet often removes foods without a clear plan for what comes next. This approach is built around identifying the specific trigger and repairing the environment that made it a problem in the first place, with a structured path back to a fuller diet.


What's a reasonable first step if this sounds like me? 

Start by tracking what you eat alongside your symptoms, including timing, rather than guessing from memory. That pattern, combined with your history, is usually enough to point toward a reasonable next step.


Putting It Together


Food sensitivities and intolerances can look similar on the surface. But the mechanisms are different.


Understanding those differences often changes what actually helps.

Because sometimes the missing piece is not the food itself. It is understanding the environment in which that food is interacting.
